Appointments

Please let us know if you think you need to be seen urgently and you will always be given priority, or in an emergency you go can go straight to the hospital or call an ambulance (000).

Longer consultations are available: please advise the reception staff if you require extra time. Walk-ins are offered appointments subject to availability and urgency as assessed by our triage protocols.

 

Home Visits

Home visits are available for regular patients whose conditions prevent them from attending the surgery. Please note that a prior arrangement is required for this.

Fees and Billing Arrangements

We are a Bulk Billing Practice. Patients without a Medicare Card will be charged a practice fee that is payable at the time of consultation. (Please note; we do not have eftpos facilities)

Communication / Telephone Policy

If you call and wish to speak with a Doctor/Nurse, reception will take your details and the doctor/nurse will return your call. We will only interrupt a consultation if the situation is urgent.

For privacy reasons, we do not provide clinical advice, book appointments or send patient information via email. Our email is only checked periodically, so for any other urgent matters it is best to call us for prompt attention.

Test Results

If you have been referred for a test, please ensure you contact us for the result within a week, or the timeframe that your doctor has advised. Results will not be provided over the telephone and patients should make an appointment to discuss results and any required treatment with their doctor.

Reminder system

Our practice is committed to preventative healthcare. You have the option of registering to receive healthcare reminders that are appropriate to your care. You are free to opt out at any time by informing your doctor or our nurse.

Patient Rights

All patients have the right, and are encouraged, to participate in decisions about their healthcare. Please ask questions about your healthcare and address your concerns in your appointment. For privacy reasons, we do not offer advice or address clinical concerns via email or over the phone.

Referrals/Engaging with Other Services

Our practice regularly engages with local health services, such as specialists, allied health and hospitals. If required, your GP will provide sufficient information (a referral letter) to plan and facilitate optimal patient care.

Patient Feedback

Please leave a comment in our suggestion box if you have a suggestion. If you are unhappy with the service you have received, please speak to your doctor about it or write a complaint to the practice manager about your concerns.

If you feel our practice has not properly dealt with your complaint, you may contact The Government of Western Australia – Health and Disability

Access to medical records:
In accordance with the Privacy Amendment (Private Sector) Act 2000, you are entitled to access personal information about yourself held by this organization. Fees apply.

My Health Record: your health information is stored securely in electronic
form and not shared with any third party without your prior consent. It may be included in registers relating to your health conditions,
i.e. Bowel cancer register, Diabetes Register, Cervical Cancer Register and so on.
Communication and access for patients with special needs includes:
Disability access to the facility, National Relay Service (NRS) for the deaf. Translation and Interpreter Services (TIS) for patients from a Non-
English speaking background.

Services not bulk billed includes; Medical Insurance, Commercial Driving Licence, Pre-Employment, Cadet Medicals,  ( cash payment required on the day)

Pre-Employment = $100.00

Commercial Licence =$130.00

Level B = $60.00 After Hours Level B = $70.00

Level C = $95.00 After Hours Level C = $115.00

Level D = $140.00 After Hours Level D = $155.00
